a b c Note: Before signing the DPA card, fill out the entire card (including the names, addresses, and telephone numbers of your health-care agents). You should sign the card in the presence of two witnesses. You may choose any adult to be your agent, but it is recommended that you not choose your physician, any of your physician’s employees, or any employee of a hospital or nursing home where you might be a patient unless the individual is related to you by blood, marriage, or adoption.
